Treatment of Parkinsonian syndrome
1. Big picture
The treatment of Parkinsonian syndrome depends on the cause. The most important form is idiopathic Parkinson disease, where treatment aims to restore dopaminergic activity in the basal ganglia.
The key exam sentence:
Parkinson disease is treated by improving dopaminergic transmission, reducing relative cholinergic overactivity, treating motor complications, and maintaining function with rehabilitation.
The main treatment groups are:
- Levodopa with peripheral dopa-decarboxylase inhibitor
- Dopamine agonists
- MAO-B inhibitors
- COMT inhibitors
- Amantadine
- Anticholinergics
- Advanced device/surgical therapies
- Physiotherapy, speech therapy, psychological and supportive care
Important exam trap:
Treatment of Parkinson disease is not only “give levodopa.” The goal is stable dopaminergic stimulation and individualized long-term control of symptoms and complications.
